Monday deadline for Pulse victims' claims

The deadline is Monday for families and survivors to file a claim in the Pulse nightclub shooting for a share of the $23 million in the OneOrlando fund

Forms are available in two languages -- English and in Spanish. The forms need to be notarized and mailed to the following address and postmarked by Sept. 12:

The city of Orlando reports that 150 claims have been filed.

The  June 12 attack at the Pulse nightclub killed 49 people and injured more than 50 others, making it the deadliest mass shooting in modern U.S. history. The gunman, Omar Mateen, was killed by police after a three-hour standoff.
